Know What Root-Cause Discipline Looks Like in Practice
Root-cause discipline means asking why until you reach something you can change: a design flaw, a maintenance practice, an operating habit or a supplier issue. Methods such as five whys, fishbone diagrams or fault trees help, but the real discipline is refusing to stop at the first plausible answer.
Write up each investigation briefly, even for small failures: what happened, evidence, causes, actions and who owns each action. Track actions to completion and check later that the problem did not return. Preserve Evidence From the First Minute
